Honestly this is quite frightening to me. Especially with all the private equity gobbling up Emergency Rooms and rearranging staffing with NPs and PAs. I'm not an ER physician but I think about this all the time. If myself or my family member were to need true emergency care whose actually delivering it. Patients have no idea who they are seeing. We have to start somewhere. I know Physicians for Patient Protection https://www.physiciansforpatientprotection.org/ is an organization who advocates for this, but there is so much more at the root of this. I feel like those in the ER trenches are best suited to comment.

How do we change this?

There have been a multitude of proposed solutions from personal coaches, to increased autonomy, transitioning to private practice to leaving medicine all together.

As a pediatrician, I deal with vaccines all day every day. I am not old enough to remember when the polio vaccine first became available, but I can imagine that the excitement for it was similar to the excitement for the Covid vaccine's unveiling. Like many others, I have signed up to get the vaccine and am trying to wait patiently (which is hard because I am so excited!). Vaccine science will be forever changed with the advent of this mRNA vaccine. As a doctor who focuses on prevention, I cannot wait to see what the future holds!
